All plans require initial three months payment upon initiation.

Monthly Subscription - one patient : $ 160 per person per month,

$ 1700 if paid in total for the year.

$120 per month for each additional family member.

$ 1000 per year for family members if paid in total for the year.

DPC for Employers / Small Businesses

Less than 5 employees : a la carte pricing.

6 - 20 employees : $ 120 per month.

21 - 50 employees : $ 100 per month.

More than 50 employees : call us.

Ask for 40% off some a la carte prices for DPC subscribers.

Botox : $12 per unit, $ 8 per unit for DPC subscribers.

Weight Management : included with any subscription.

BHRT : Bioidentical Hormone Replacement Therapy, multiple hormone replacement :

$130 per month without DPC subscription,

$10 per month with DPC subscription.

We provide two months advanced notice to our clients on all pending price changes.
